NSU Health accepts a wide variety of insurance plans at our many clinic locations. The list of insurance plans & managed care plans that NSU Health providers are contracted with is for reference purposes only and is subject to change. Check with your health plan to confirm that your NSU provider is contracted. NSU Health may participate in some, but not all, products offered by a health plan. Coverage limitations are dependent on individual group contracts.
If your health insurance provider does not have a contract with NSU Health, this means you may have out-of-network, or non-contracted insurance plan and you will be billed for all services or pay out-of-network rates.

At the time of your appointment, please bring photo identification and your insurance cards. Pre-authorization for medical services is required by many insurance plans, as well as HMOs. We highly recommend consulting with your insurance company or primary care physician to determine if pre-authorization is required prior to the date of your appointment.

Additionally, if you choose self-pay, you may also qualify for a reduced rate (application and approval for financial hardship required).
