Please carefully review the following terms and conditions (the "SMS Terms"), which NSU Health may revise at any time by updating this document on its applicable website. If you use, partake, send, or receive SMS/text message with/to/from NSU Health after such revisions have been made, you will be bound by the revised SMS Terms.
By agreeing, consenting, opting in, or checking the box to sign up for one or more text messaging programs, you expressly consent to receive electronic communications to your mobile number from NSU Health and its employees, agents, assigns, vendors, and affiliates including, but not limited to, text message/SMS, and other means of electronic messaging/communicating with respect to:
- (i) services or goods requested or received by me (or a minor who I am the legal parent/guardian of, and I have authority to provide such authorization and consent on behalf of the minor);
- (ii) all other NSU Health services or goods;
- (iii) post-care surveys and feedback;
- (iv) healthcare related notifications and information; and
- (v) for any other matters including NSU Health’s marketing and commercial efforts.
Consent to receive marketing text messages is not required to purchase any goods or services or receive treatment. Your opt-out request may generate one final message confirming that you have been unsubscribed. Message and data rates may apply depending on your text/data plan. Your wireless carrier and other service providers may also collect data from your SMS usage, and their practices are governed by their own policies.Please contact your wireless provider for more information. NSU Health and carriers, like Verizon, are not liable for delayed or undelivered messages. Message frequency may vary.
